Self Reflection: Behavior Growth Survey

This is to be administered to students that have shown great improvement behaviorally. This is an independent survey. This should not be released to students in the beginning of the year unless it is presented with the understanding that the student has shown growth from the previous year. You usually want to give enough time in the beginning of the year for students to grow into their changes so I suggest waiting until after Quarter 1/Semester 1. There is a section in the survey that asks for students to write on a sticky note. Please provide sticky notes ahead of time next to the computer the student is using. The sticky note serves a really important purpose as I would create a wall with sticky notes of reallife strategys from students that have helped them improve. This is great 2 fold. 1.) This lets me know what inspired my students to work on what they were having struggles with 2.) The sticky notes are visible for all students to see. The message may motivate a student to think of a new way to solve a problem. Also, the student reading the sticky note may be motivating by the peer that wrote the message and their courage to share can strengthen the struggling student to push through.
